3) Enter publication year.

4) Enter title of measure in appropriate field.

5) Enter a brief description of the measure in the appropriate field.

6) Enter the instructions for administering the measure in the appropriate field.

7) Enter publications and presentations demonstrating validity and reliability of measure in the appropriate field.

8) Enter other publications and presentations in the appropriate field.

9) Select whether the submission is a 'first time submission' or a 'modified resubmission.'

10) Attach the measure as a PDF file.

12) Submit (SAVE) the form.
